Hello, I'm Nurse Carrie, I have been a Pediatric nurse for 28 years. I'm married and have 2 fur babies Izzabella and Charlie. I enjoy helping everyone be and stay healthy, My office is a safe place to come for any reason if you are needing help because you don't feel well or just need a time out. I enjoy traveling and baking and just hanging out. I look forward to helping you stay healthy so you can be successful.

Campus Health

In an effort to prevent the spread and control of communicable disease, parents should use the following criteria to decide when to keep their child home from school:

  • Fever 100.0 F or greater
  • Body aches/ Chills
  • Cough
  • Sore throat
  • Severe headache
  • Rash
  • Nausea/Vomiting/Diarrhea
  • Conjunctivitis (Pink Eye)

The student should remain at home if symptoms are serious enough to interfere with their ability to concentrate and learn. The student may not return to school until he/she is fever free for 24 hours without the use of fever suppressing medications. If the fever persists for more than two days or symptoms become worse, please consult with your health care provider.

Keep your emergency contact information current and updated

Report illnesses and injuries to the school nurse, such as: Covid-19, Flu, Strep Throat, Chickenpox, Fractures, Sprains, etc.

Also, report any cases of Head Lice.

Please drop off a copy of your IMMUNIZATION RECORD or EXEMPTION to the Nurse's Office. If you have any questions or need any help please feel free to call 817.817.4500.

FWISD Medication Administration Forms:

If possible, all medications should be given outside of school hours. Three times a day medications can be given before school, after school and at bedtime.

Medication Administration Request Form (Eng/Spa)
Use this form when the nurse is to administer oral or topical medications to your student.

Specialized Health Care Procedure Authorization Form (Eng/Spa)
Use this form when the nurse will be performing a specialized procedure such as nebulizer treatment for asthma student, or injecting an EPI Pen, etc.

NOTE: All medication submitted to the Nurses Office must be in original properly labeled containers from the pharmacy. The pharmacy can supply you with 2 labeled bottles for this purpose. Medications sent in baggies or unlabeled containers will not be given.

Screening Information


Acanthosis Nigricans Screening pupils in grades 1, 3, and 5.
Acanthosis Nigricans (AN) is a physical skin finding that may be a marker for high blood insulin levels (hyperinsulinemia), suggesting insulin resistance, that is associated with obesity. The goal of this screening is to identify children with AN and intervene to reduce their risk of developing diabetes.

Hearing Screening pupils in grades K, 1, 3, and 5.
The results of the pure-tone audiometric Sweep-Check Screen are recorded for both the right and left ears. The Sweep-Check Screen is performed at an intensity less than or equal to 25 dB at the following frequencies: 1000, 2000, and 4000 Hertz. The goal is to identify children hearing problems early and refer them to appropriate remedial services.

Vision Screenings pupils in grades Pre-K, K, 1, 2, 3, 4 and 5.
For vision screening a distance acuity for the right and left eyes is recorded. At Manuel Jara Elementary, Alcon Vision center has partnered with FWISD for eye exams for your child, at no cost.  Parents must sign consent. The goal is to identify children with vision problems early and refer them appropriate remedial services.

Spinal Screening pupils in grade 5 (10 y/o girls).
The goal is to identify children with scoliosis early and refer them to appropriate remedial services.
